Employee Birthday Donation Program
Our Employee Birthday Donation Program allows employees to select any Non-profit (501c)
Charity to which the bank will donate $200 on the employee’s behalf during the month of their birthday! This has been an amazing way for our employees to give back to causes that are close to their hearts.

If you would like your organization to be considered for a donation or sponsorship, and the amount being requested is less than $1,000, please complete and submit the forms below with a W-9 for your organization in one of the following ways:

Note: Donations requests are accepted from organizations located or performing services in First Community Bank's Northern Utah footprint in the following counties: Davis, Weber, Morgan, Box Elder and Summit.
